Consent for Nursing Services

All enrolled students must have a nurse consent form filled out each school year in order to receive non-emergency services. Students are given these forms at the beginning of each school year and parents/guardians are asked to please fill out the requested health history and sign and return the form to your childs school. Your signature on the form gives your written permission for your student to be assessed and treated by the nurse. Important: all over the counter medications for non-emergent complaints will require a form on file with a signature from the parent before authorized medication can be administered. For student safety no phone calls authorizing medications will be accepted.
