To apply for the McKinney-Vento Homeless Education Program, please print and complete a McKinney-Vento eligibility form by clicking on the link below.

Completed packets can be returned in the following ways:

• By U.S. Mail to:  Title I/McKinney-Vento, 484 Mulberry St Suite 280, Macon, GA 31201
• In Person:  Deliver the forms to the McKinney-Vento Office (same address as above)
• By Fax to:  478-765-8651
• Return to the school counselor
